How to Apply a Hollister 14103 Skin Barrier With Tape Border?

  1. To change a two-piece pouch first you will want to get your supplies ready. You will need the following items to prepare a new pouch and put it on your new skin barrier. Your new pouch and a disposable bag for used pouch, water, and some tissues or paper towels for washing and drying skin.
  2. If you are using ostomy accessories with your pouch, you will need to get those out and ready at this time
  3. After you have assembled all your supplies, you are ready to prepare your new pouching system
  4. To prepare the skin barrier, first remove the release liner on the skin barrier
  5. Then stretch the form of Flex shape to fit the starter hole to the size and shape of your stoma. The skin barrier has ridges designed to help make it easy to shape and reshape. Do not roll the barrier just stretch it until the opening is approximately the size you need set the form of the flex skin barrier aside ensuring the adhesive side is facing up
  6. Now is also a good time to close your drainable pouch, if you are using a pouch that has an integrated closure like 'lock and roll', close it now
  7. If you are using a pouch clamp, apply that now that you have prepared your new pouching system. It is time to remove your used skin barrier and pouch
  8. It is easiest to empty your pouch before you remove it. When taking your pouch off, carefully lift the edge and peel downward, gently pushing the skin away from the adhesive
  9. You can use one of the convenient disposal bags included in every box of pouches, these bags allow discreet disposal of used pouches properly dispose of your used pouch. If you are using a pouch clamp, remove it before you throw away the used pouch. The pouch clamp can be reused.
  10. Clean your skin with water and if needed a gentle soap. Do not use oils powder lotion or lotion types soaps on the skin
  11. Rinse and dry your skin thoroughly
  12. Center the skin barrier opening over the stoma in a diamond-shape
  13. Now gently push in the form of a flex skin barrier towards the stoma to help ensure no skin around the stoma remains exposed
  14. Gently press the barrier to be sure it makes good contact with the skin
  15. Remove the backing paper of the tape and press the adhesive against your skin
  16. Attach the pouch to the skin barrier by engaging the lower edges of the pouch flange and the skin barrier flange. Press the two flanges together a series of clicks will confirm you are doing it correctly. No more clicks means the pouch is secured.
  17. Place your hand over the pouch and barrier and gently press for about one minute. Warmth and gentle pressure help activate the adhesive.
