I have the AR001. It has a plug on it. Do I put water in it before microwaving it, and if so wouldn't the water get mildew  leaving the water in it.

The AQUA-RELIEF Pads are cushioned with internal foam and are simply filled with hot water or microwaved before use. 

So if you are putting the pad in microwave then there is no need to put water in it.

On one reply below you say you do not have to fill with water, the one right below it says 2qrts of water are needed to saturate the foam before microwaving. Which is it? Mine has never been used 
. Bought it in 2008 and can't find instructions

Unfortunately instructions are not available for Aquacel Hot and Cold Relief Pad. 

But water is not required in this pad. 
For hot therapy just put it in microwave and put in freezer for cold therapy.

You need not have to put water inside the pad.
For hot therapy put it in Microwave and for cold therapy put it in refrigerator.

You put in about 2 quarts of water making sure the foam pad is saturated. You will have to choose a temperature that is comfortable for you.

I've lost my instructions. What is the recommended time to heat in a microwave of 1200 Watts? The product says to not "overheat" but gives no guidance for what constitutes overheating.

The recommended time to heat the pad is 1-2 minutes, test it, flip the pad and heat an additional 1-2 minutes. Then if needed in 30 second increments to reach the desired heat.

How do I receive a return authorization number?

How long does it take to receive a refund?
